Dr. Bauback Safa, MD is an internationally renowned Reconstructive Microsurgeon at the Buncke Clinic in San Francisco, widely considered to be the birthplace of microsurgery. Dr. Safa serves as Clinical Assistant Professor of Surgery at both Stanford and UCSF divisions of plastic surgery. He is highly sought after as a surgical educator and has trained hundreds of surgeons around the world in all aspects of microsurgery.
Dr. Safa is also is the Medical Director of the San Francisco Transgender Institute, a multi-disciplinary center that provides comprehensive care to transgender and non-binary patients. He and his team have performed more than 700 Phalloplasty surgeries since 2012, making him one of the most experienced Phalloplasty surgeons in the world.
Dr. Safa obtained his medical degree and completed his residency in plastic and reconstructive surgery at Stanford University. He was subsequently fellowship trained in microsurgery and hand surgery at the Buncke Clinic where he has been on staff since 2007.
Dr. Safa is board-certified by the American Board of Plastic Surgery (ABPS) and is a member of the American Society of Plastic Surgeons (ASPS), the American Society for Reconstructive Microsurgery (ASRM), the World Professional Association for Transgender Health (WPATH) and numerous other surgical societies.

Dr. Safa is affiliated with California Pacific Medical Center, the largest medical center and flagship hospital of the Northern California-based Sutter Health system, and Kaiser’s San Francisco Medical Center.
Surgery Requirements
Dr. Safa follows WPATH guidelines for surgery:
- You will need to provide two referrals from qualified mental health professionals.
- A letter from your hormone specialist stating you have been on 12 months of continuous hormones, unless clinically contraindicated.
Per hospital policy, patients must be 18 years of age or older.
Financing & Insurance
Dr. Safa accepts insurance from a wide number of insurance companies, including Blue Shield, Blue Cross Blue Shield, United Healthcare, Cigna, Kaiser and Health Net, as well as Medicaid (Medi-Cal). A cash pay rate can also be requested.
