Dr. Drew Marano, MD is a highly skilled plastic and reconstructive surgeon in New York City specializing in Gender Affirmation Surgery. Born in New York City and raised in New Jersey, Dr. Marano is very pleased to join Dr. Jonathan Keith at East Coast Advanced Plastic Surgery. Dr. Marano is fellowship-trained in gender-affirming facial, chest and genital surgeries, including Phalloplasty and Vaginoplasty. Dr. Marano is a classically-trained pianist and earned his bachelor’s degree in Music from Brown University. He subsequently graduated at the top of his class from Rutgers New Jersey Medical School. Dr. Marano completed his six-year Integrated Plastic and Reconstructive Surgery Residency at the prestigious Columbia University / Cornell University program in New York City. Following residency, he pursued further specialization in Gender Surgery, becoming the first-ever Harvard University Complex Gender and Microsurgery Fellow.
Dr. Marano has co-authored numerous peer-reviewed academic papers related to Gender Surgery. His latest research is focused on social and systemic barriers to care, scarcity of providers and formalized training, and advancements of surgical techniques.
Masculinizing Surgery
Dr. Marano is experienced with all types of Top Surgery, including Double Incision and Keyhole. Patient preferences and chest anatomy determine the best approach. Dr. Marano will discuss important anatomical factors to consider in choosing a method, such as skin elasticity, size of the nipple-areola complex, and chest size and shape.
Dr. Marano also offers gender-affirming genital surgery. Radial Forearm and Anterolateral Thigh are the most common donor sites for Phalloplasty, however Dr. Marano can also use the MLD, abdominal or groin donor sites to best suit a patient’s needs. With both Metoidioplasty and Phalloplasty, Dr. Marano uses rotated labial flaps for Scrotoplasty and requires Vaginectomy if urethral lengthening is also being performed. Patients can choose between saline- or silicone-filled testicular implants, and semi-rigid/malleable or inflatable penile implants.
Feminizing Surgery
For those on the trans feminine spectrum seeking genital reconstruction, Dr. Marano offers testicle removal (Orchiectomy), creation of a vulva without a vaginal canal (Vulvoplasty), and creation of a vagina (Penile Inversion or Peritoneal Vaginoplasty methods). Hair removal is required for Vaginoplasty and Dr. Marano’s practice has a certified laser technician in-house.

Surgery Requirements
Dr. Marano is a member of the World Professional Association for Transgender Health (WPATH) and follows WPATH guidelines for surgery:
- You will need to provide two referrals from qualified mental health professionals for genital procedures; one referral for other procedures unless otherwise specified by insurance.
- A letter from your hormone specialist stating you have been on 12 months of continuous hormones for genital procedures, unless clinically contraindicated.
